I had rough running on cold start, albeit seemingly not as severe as your issue. Check viewtopic.php?p=676639#p676639, ended up being a bad VVT solenoid causing a code, which I never cleared.

I try to keep it quick on VIDA for that reason. While initiating VIDA you need the key in position II, but after you identify the car and such you can turn on the engine to help keep the battery charged. Also as soon as you plug it in to the port it is powering it so have VIDA booted on your ...

It turns out it was an un-cleared ECM-644A code. I had replaced the VVT exhaust solenoid, had the t-belt done, and cam seals done back in the fall/winter. With the reset of the code 3 days ago, the vehicle has a normal cold start. This includes the initial rev to ~1500rpm before returning to ...

I just checked my VIDA today after getting the t-belt changed on my 2001 V70 T5 around new years. The ECM-644A code was still there, and it was around prior to t-belt replacement, solenoid, and hub replacement. Cleared it off and has not returned after some spirited driving. Supposedly was ...

Probably a loose connection. I had a similar issue and was able to get my hand in behind the housing and jostle it around a bit. That was a few months ago, has not been an issue since. You could also clean the connection surfaces off with some alcohol, if that doesn't do it.

I had, perhaps, a similar issue to what your describing. Mine seems to be more weather-based as the idle fluctuation on cold starts has subsided with the summer warmth. Here is a link to my previous post. Maybe you can draw some similarities. (Videos attached in it)
