Inserting the extension is designed to hold the crank in position, correct. However precopster is suggesting you use the extension for a related purpose, not to hold the crank, but as a method to determine if the crank is in the right place. The extension will not slip in until the crank is on the mark.
He is right about the importance of the crank location and which crank mark you are using: it is absolutely essential the the crank be in the right place. You can confirm that by pulling the starter as he suggested, or by various other methods (two other examples (1) confirm the marks, e.g. post up a pic here; (2) use something like a long zip tie in #1 to confirm that it is almost cresting when the crank is on the mark). The crank mark can be hard to see sometimes, and you don't want to be wrong so he is trying to be super careful about it.
