How many miles? How has the transmission been operating? How is the fluid color and smell? Any transmission repair/replace history?

My thoughts going off what RickHaleParker states:
Glycol contamination - check both fluids tranny and coolant. Have an engine coolant pressure test. If the system is compromised this should confirm. Some shops rent pressure check tools.
Solenoid and internal leakage in transmission - this sounds most likely a valve body issue. At your miles it would not be a surprise, but I am uncertain what transmission you have in your car. Also what scanner are you using?
As your fluid is visibly looking good I would discount worn clutch plates - a history of black fluid is what one would expect.
